What This Means for Your Practice
How Dollar Based Dental Works for Providers
No Network Contract Required
Dollar Based Dental is not a traditional dental network. You do not need to sign a participation agreement, accept a fee schedule, or join a network to serve Dollar Based Dental members. Any licensed dental provider may treat members.
Treatment Freedom
Clinical decisions stay between you and your patient. Dollar Based Dental does not impose network contract limitations on treatment recommendations. You recommend the care that is right for the patient — benefits are determined by the employer's plan documents.
Standard Claim Submission
Claims are submitted using the standard ADA dental claim form to Eagles, the plan's third-party administrator. No proprietary claim forms, no special credentialing, and no network enrollment required.
Serve More Patients
Dollar Based Dental members are not restricted to a network. Patients who carry Dollar Based Dental coverage can choose you as their provider regardless of whether you participate in any traditional dental network.
Step-by-Step
How Claims Work
The process is straightforward and uses the same standard ADA claim form you already use for other plans.
Patient Presents ID Card
The member presents their Dollar Based Dental participant ID card at the time of service. The card identifies the plan and includes contact information for Eagles.
Verify Benefits Before Treatment
Contact Eagles prior to treatment to verify the member's benefits, annual maximum, deductible status, and covered services under the employer's plan. This step is strongly recommended before major treatment.
Complete Treatment
Provide care as you normally would. Treatment decisions are between you and the patient. Dollar Based Dental does not require pre-authorization for most services, but check the plan documents for any specific requirements.
Submit Claim to Eagles
Submit the completed ADA dental claim form to Eagles. You may submit on behalf of the patient (assignment of benefits) or the patient may submit after receiving a statement. Eagles contact information is on the participant ID card.
Receive Explanation of Benefits
Eagles will process the claim per the employer's plan documents and issue an Explanation of Benefits (EOB) to both the provider and the member. Payment is made according to the plan's dollar-based benefit structure.
Claim Submission Address
Submit completed ADA dental claim forms to Eagles. Contact information for MediConnx is printed on the member's participant ID card. For benefit verification prior to treatment, call the number on the ID card.
MediConnx is the third-party administrator for Dollar Based Dental plans administered by Eagles, Benefits by Design, Inc.
Clinical Independence
Treatment Decisions Stay Between You and Your Patient
Traditional dental network contracts can create tension between what a provider recommends and what a carrier's fee schedule or coverage policy allows. Dollar Based Dental removes that dynamic.
Because Dollar Based Dental is a self-funded employer benefit plan — not a carrier-managed network — there is no network contract influencing your clinical recommendations. You and your patient decide on treatment. Benefits are then determined by the employer's plan documents.
What Providers Should Know
Common Questions
Provider FAQ
Do I need to join a network or sign a contract?
No. Dollar Based Dental is not a traditional dental network. There is no participation agreement, no fee schedule to accept, and no credentialing process required to serve Dollar Based Dental members.
How is payment determined?
Payment is determined by the employer's plan documents, which define covered services, dollar-based benefit maximums, deductibles, and any exclusions. Payment is not based on a network fee schedule or UCR table.
Where do I submit claims?
Claims are submitted to Eagles, the plan's third-party administrator. Use the standard ADA dental claim form. Contact information for Eagles is printed on the member's participant ID card.
Can I verify benefits before treatment?
Yes, and we strongly recommend it. Contact Eagles before major treatment to verify the member's benefits, annual maximum, deductible, and covered services. This helps avoid surprises for both you and the patient.
What if I have never heard of Dollar Based Dental before?
That's common. Dollar Based Dental is a self-funded employer benefit plan administered by Eagles, Benefits by Design, Inc. It operates similarly to other self-funded ERISA plans. The member's ID card includes all the information you need to verify benefits and submit claims.
Is there a fee schedule I need to follow?
No. Dollar Based Dental does not impose a network fee schedule on providers. You bill your standard fees. Benefits are paid to the member (or assigned to you) based on the employer's plan design, up to the applicable dollar-based maximum.
